To make a grant distribution from your Donor Advised Fund, download a PDF of the Donor Advised Fund Recommendation Form, then complete all parts of it, sign and date it, and return it to: ATTN: Joan Elder, Program Officer, The Springfield Foundation, 4 West Main Street, Suite 825, Springfield, Ohio, 45502. The form may also be faxed to 937.324-1836.
